Patient Info

We accept most major insurances insurance plans and HMO's. Be aware that many of these require a referral from your primary care provider. So before making an appointment, consult your insurance plan handbook and then contact us.

Please remember to bring the following with you to your appointment:

  1. Insurance card
  2. Referral (if necessary)
  3. A list of ALL medications with their dose and frequency
  4. Pertinent medical records
  5. MRI, CAT scans, or laboratory test results available
  6. Reports of psychological, educational evaluations
  7. Academic records and/or special services records
  8. As a parent, please recall how your child advanced in: sitting up, walking, talking, learning.

Please take time to print and to fill out the pertinent forms and screening questionnaires listed below (depending on which condition you seek evaluation for) and make sure to bring the materials along with you to our meeting.
